Existing Primary Members may use this form to start Direct Deposit of part or all of their paycheck into their Bay Cities account.

Complete part 1 and submit it to your payroll department. Complete part 2 to submit your request to Bay Cities. Your Direct Deposit will not begin until both portions of the form have been submitted.

I have printed and filled out part 1 of this form. I understand that my Direct Deposit will not be processed until I have turned part 1 in to my employer's payroll department, and completed and submitted part 2 to Bay Cities.
